To ensure we can provide your care safely and effectively, we need to be certain that we have correctly addressed your needs and wishes, answered your questions and also have taken account of your medical history and any treatment that you are undergoing as this may affect how we can provide care for you safely.   
Your care may involve several members of our team at this practice and we want to assure you that all members of our team will respect the confidential nature of the information that you have provided and will always protect it from being used by others in an unauthorised way. 
We will ask you to complete in writing how and in what circumstances we may use this information in future.

We may receive information about you from a referring practice

If you permit other organisations to give us information such as if you apply for a loan in connection with treatment

The type and quantity of information we collect and how we use it depends on why you are providing it.
Typically, the basic personal information we collect might include:

    • Your GP name and practice address
    • Your next of kin or carer contact details

We process and use your information to provide you with safe dental care.
Disclosure will take place on a ‘need-to-know’ basis, so that only those individuals/ 
organisations who need to know in order to provide care to you and for the proper
administration of Government (whose personnel are covered by strict confidentiality rules will be given the information.  Only information that the recipient needs to know will be disclosed. Such as

The hospital or community dental services
Other health professionals caring for you
NHS payment authorities
The Inland Revenue
The Benefits Agency, where you are claiming exemption or remission from NHS charges
Private dental schemes of which you are a member.

 be disclosed to a third party not connected with your health care.  In all other situations,

4.2 How long do we keep this information?
Retaining information: We are required to retain your dental records, X- rays and study
models while you are a patient of this practice and after you cease to be a patient, for at least
eleven years or until age 25, whichever is the longer. 
 
4.3 How do we keep this information safe?
When you give us personal information, we take steps to ensure that there are appropriate technical measures in place to protect it. For example, access is restricted only to those who are entitled to see the data, our online templates and audits are always encrypted and our network is protected and routinely monitored.
Your information is held in the practice’s computer system and  in a secure manual filing system.  The information is only accessible to authorised personnel. 
Personal information will not be removed from this practice without the patients authorised consent. Your personal information is carefully protected by the staff at this practice.
All access to information is held securely and can only be accessed by regularly changed passwords. Our online templates and audits are always encrypted and our network is protected and routinely monitored
 

5. Who has access to this information?

5.1 Your Rights
You have a right to ask us to stop processing your personal data and, if it’s not necessary for the purpose you provided it to us (e.g. to provide you with the services, products or information you asked for, processing your payment) we will do so.
